The Office of the Registrar of the University of San Jose-Recoletos (USJ-R) acknowledges its responsibilities pursuant to Republic Act No. 10173 or the Data Privacy Act of 2012 (DPA) with regard to the collection and processing of personal data of its data subjects (students), and further ensuring that all information collected from the students are processed in adherence to the general principles of transparency, proportionality, and legitimacy of purpose. Also, this Office recognizes the rights of the students to: a) be informed, b) object, c) access, d) correct, and e) the rectification, erasure, or blocking of personal data as long as it is within the bounds of the DPA and University policies.
The personal data obtained from the students is processed, entered and stored indefinitely within the University’s authorized systems and processes for documentation and program administration and will only be accessed and used by authorized personnel and clerks of the Registrar’s Office.
Declaration of Consent:
In view of the foregoing and in connection with my enrollment with the University of San Jose-Recoletos, I hereby give my consent to USJ-R to collect, use, store, update or modify, process, disclose or share my personal data such as but not limited to the following sensitive personal information – name, age, date of birth, civil status, religion, address, contact numbers and other contact information, education records – as requested by its proper authorities and subsidiaries. Likewise, I am giving my consent to the University to share my personal data for reporting purposes required by government agencies (e.g., CHED/DepEd, PRC, CSC, LEB, TESDA, BI, DFA, NICA, etc.) relative to my enrollment/graduation.
I also give my consent to USJ-R to have my photographs taken during school-organized events/activities. The University may use these photographs for newsletter, email communications, marketing and/or website publications, yearbook, and for any other legitimate purposes and intents of the institution.
